I am not sure just how to fill this ballot form for the commons It also has a form W9 with questions Im not use to seeing. Can someone post a simple step by step of each page of the TDAmeritrade ballot form. And where it gets sent to. Also I have not received anything for the PIERS Yet. And I have some Ps and Ks. Could someone tweet Ilean and let her no that no ballots received for these other shares.
