Just call 1-800-etrade-1...get through to a rep for corporate actions...may take a little explaining to the first customer service person you get.

But when you get the right person, they'll ask if you're going to accept or not...opt in or not...ask for a confirmation number...hold on to your hard copy materials(ballot & w-9)in case the fed wants it later...

Should see your cusip change on your account when the shares are locked....
