I'm not sure of the process but Etrade sent a "big cert" to the ta. The ta put the number of certs in my name that I requested. Sent both certs back to etrade and etrade sent me my cert. This was explained to me by etrade. I asked the ta to send it to me but he said that etrade had to send it to me.
