OK...unless they filled out the form for you, which is a possibility there is nothing in the information memorandum that would lead me to believe that there is an automatic transfer.
WE will received the UNIS shares--but who knows when. Those who used "favored brokers" got the stock when the shares available for trading were few--meaning they could sell for a huge profit. Customers of online discount brokers apparently have to wait until everybody else get their shares--the little guy gets screwed again!
