You clearly forget the trial lawyers. Willie Geary went to court to enforce his bill that worked out to $11,000/hr and this was granted by the judge.

What is billed in medicine and what is collected are so far apart it is almost meaningless. The ER bill can be 4 grand and the insurance will only pay the hospital $500.

An angle I'm seeing more and more of is bios with late stage products discussing how doctors will be motivated to use because they'll be compensated per procedure. This question is coming up more, ok, great product, but will doctors get paid and be properly motivated? Saw one company recently with a non ideal delivery (intravesical-direct to the bladder) but investors lining up to go in because the drug probably works and docs are going to get paid, so surely it will do well! :)
