serf, it all starts with the broker, at least in my case. I started by speaking with BofA's Priority Response team. They faxed me 144 forms (and a stock power form), I got medallion signatures from a local bank, then overnighted the signed forms back to BofA.

Next step, per BofA's instructions, was to send the $300 payment to Paivis Corporate Counsel (Gregory Bartko). I called Bartko's office at that point to confirm the info and that I was mailing the check to the right place. Their phone number is 404.238.0550.

So it all started with a request to file 144 with BofA.

With BofA it was a very transparent and easy-to-follow process. No charge by BofA. Only charge was the $300 to Bartko. No reason it should take 4-6 weeks to file 144 through Ameritrade or another broker. It took less than 1 week. Actually, I overnighted my $300 to Bartko late on Friday, received there in Atlanta on Monday. So from that step to today it was 3 business days.
