She told me on the phone today they will not sign the forms. Supervisor is Angela Chisholm in Reorganization and Safeguarding Dept.

My broker gave me a phone call to tell me they couldn't do it. In the end....they did do it but to my knowledge they have not mailed it yet? They did send me a pdf file of the filled out form.
