There has been no straight answer. My guess is this.... Those that send in the $20 will get their dividends in cert form. Those that don't will have to wait for them to get credited to their accounts. I've sent an email to the company as per their request, and have not heard anything back from them. I'm not going to some website and downloading a form. If the company can't email it directly to me, then it must not be that important. The dividend was officially PR'd, so it must be given out. I have heard a few here say their brokerages have stated that it will show up in their accounts 5/31. I'm going with that.
