Rett is Genetic, Not a Geriatric Disease

A 'reset' is quite possible; however, I recall the Rett(?) trial where patients worsened during the drug "holiday", but resumed improvement once blarcamesine was restarted.

That was the case. Treatment of Rett Syndrome absolutely will require continued, uninterrupted dosings of blarcamesine. Terminating them will allow the prompt resumption of symptoms.

But that's because Rett symptoms appear at birth from an inborne genetic defect, not because of any sort of aging or "wearing out." Blarcamesine will not be able to accomplish any genetic engineering. Blarcamesine will be doing many wonderful things, but not reconfiguring of defective genes.
