I have to print the form to send to my doctor to get an exemption. Funny how they did not offer lovazza or GV as an alternative. They told me the drug was dropped for one of the following reasons.
1) they did not have a supplier agreement with the manufacturer or
2) BCBS determined no benefit in taking the drug.

So first I get the form to my Dr, then I file a complaint with OPM because BCBS has the nerve to increase premiums by 15% and primary physician copays by 50% and then drop drug coverage.

They would not give me the department or name of someone I could escalate to so I am going scorched earth, OPM, Maryland AG and Better Business Bureau.
