Comparison of MGMT methylated results in the DCVAX trial to methylated ECA patients is apples to apples and not affected by different prevalence of MGMT in the samples - DCVAX still did way better. I also assume all of the things you noted were statistically adjusted in the analyses reported. Adjusting for relevant confounds is standard practice.
