And yes it is that easy, IF you are on top of it right away. Thats why companies need to know EVERY DAY where their shares are and why a good Transfer Agent is needed.

Aside from that its simple math and will show up in any proper share count.

And as for why we dont see more of it being called out, likely its just because the company is embarassed or they just dont care.
