If you have other holdings and do not plan on transferring your entire account out of TDA it's my understanding that there is no charge. Other wise there is a $75 fee for transferring out your entire account.

You'll have to print it to sign & date it. Then scan or take a picture of it and attach the image file(jpg) to your message before sending it to TDA message center.
