Pragmatic is one thing , ethical quite another.
A PREVENTION trial should first prevent death CVA MI using known beneficial IPE effects--this trial cannot ethically and knowingly withhold Vascepa from a group termed "high cardiovascular risk" with prior CAD event knowing teh results of R-IT and sleep at night...I cannot see a way forward via ethical randomization without giving Vascepa to both groups.
