I am usually for full accountability but you have to be *reasonable*. You can't always judge someone based on the outcome that they had no control over. For example, last year's Golden State Warriors - one of the greatest teams of all time. Had a fantastic regular season then in the Finals, 2 or their top 3 players got injured and they get blown out. Should their coach have gotten fired because they were blown out or do you equate those injuries to Du's erroneous ruling that Thero didn't have any control over?
If you said during the regular season that Durant has weak ankles (must be replaced) and is likely to be injured in the finals, then you should be able to say "told you so", the coach needs to be fired since he should have known this.
Same with Thero. Unless you (anyone) said before Du's erroneous ruling that Thero is doing ABC incorrectly and you would do XYZ instead, your opinion is pretty worthless after the fact.
