Ask them why the regulatory form that was submitted doesn’t reflect what they’re telling you verbally. Ask them that.

You have never stated that you asked them about the incorrect code on the form. And you took the time to write all of that and still never confirmed. So it is safe to say that you did not mention the incorrect code or ask them if they planned on refiling to change it.
