Last I knew V was out of formulary with my Kaiser in MD; persuaded my Dr.
to prescribe but I get it at CVS and pay 100%, ~$200/month after coupon.

You're not looking a Kaiser Medicare Part D formularies - I live in MD and can guarantee you Kaiser does not cover it - when I looked at plans during open season last year they didn't even have it on T4 - it wasn't listed at all. Haven't checked to see if that's changed because there's no point.
