bfost - Wow! that is quite the letter to send. Only problem is it won't make any sense to some bean counting junior level person that reviews these. They will never read that entire letter. They need to get the expanded label ASAP. I find it odd that it took nearly a year for someone to even contact you about V even if this one person had the whole state. What happened to the 400 reps that have been on the payroll ? With 400 how could 1 person be responsible for a whole state?

What amount of time does it take to fill out that form for the "average" patient? I assume of course there is an average patient---probably not the case. So....

What was the longest time it took you to fill out that template form?
What was the shortest time?

The answers( if there are any in memory) should give us non-doctor types an idea and appreciation of the amount of time it takes doctors like you to get a V scrip for patients. I see on closer reading that Amarin has provided a lot of copy and paste paragraphs for the physician to use in filling out the form.

Next time I see my own physician--who prescribed off-label V for me in early 2013--I will ask him whether a template form[like the one you provided us] even existed.

North- It probably takes an extra 5-10 minutes to personalize the template a bit although you could just copy and paste it and make it a nurse or admin function. Many offices are now using the below vendor to assist in the process. The AMRN rep said they work with the below provider to help with coverage. We use them.
