CVS/Caremark, like every other PBM, has different tiers of what they will cover depending on what level the employer is willing to pay for, its not one size fits all. If your employer wants to pay for a Cadillac pharmacy benefit you will get Cadillac coverage. If not, than your shit out of luck! I pay $30 for 90 days of Vascepa, off label through Cigna home delivery or $30 for 30 days at my local Walgreens.
