Yes they just sent it to me. I didn't request it or anything. Actually I forgot I even had the stock. I usually just throw away that kind of stuff without even opening it but decided to open it. I had a form to fill out and you could either mail it back in or email it. I filled it out and emailed it. I may could send you the info and the form if you want.
