that is not the case (at least for points 1 and 2)
1) It was Boots, and may be a requirement of prescription coverage, that decided to pull off the shelves. Not a BIEL decision. but available from multiple outlets some prescription (Lloyds Boots etc) some on the shelf and many on line anyway.
2) If you follow the facebook page you would know that each county does have its own formulary just like each health insurer here does. The NHS decision gets it put on them automatically as new device/drug. but then each county seems to have to review these new devices and agree to put on their formulary as covered and hence their doctors can prescribe and be covered. BIEL is now contacting every one of them and most have quickly addressed the problem (Lancashire and Derbyshire at least if my memory is correct). Something similar in Scotland if I recall.
Attending the next GP Conference as well where there will be better education for GP's since if the prescribe a national online pharmacy only needs them to be on the NHS list which they have been since April
