What does start the form mean? Forgive me for being cynical but I can start a form and leave it on my kitchen table for 2 years. My question is have you filled out the form and turned it in to the proper channels yet? Not started a form by filling in my name and that's where it ends. Just asking?

Wait a minute...what was all that talk of being in finalization? And that was at least a week or two ago. They have been working on this for months supposedly. Clear answers should be given. Not all this nonsense.
