Missling has touched on this several times, but I think this statement from the 38:45 mark of The Deal: Pharma Outlook for 2017 is particularly notable (rough transcript):

There’s also another element which I found in this 21st Century Act is that the requirement to have a interaction with a patient, with a sponsor and the patient, and I think it’s a good idea. It’s a bit like, you know, you’re asking your customer what you really need, what you really want, what really is needed for you to improve. And it also raises the bar for the treatment. You can not just provide an incremental measure which is not really picking up on a patient’s perception, that only has a significance, but it doesn’t really improve anything for the patient, itself. That is really important. You really want to have a treatment which improves lives, improves the patient’s perception of life, and that is the difference of innovation compared to incremental advancing of a compound.
