Camera trolley/stability and calibration procedure/refinement necessary. Multiple failures, irresponsible. Revise existing poor QC standards and supervision immediately. Perhaps require an adult to be in the room and sign off for final scenario approval. And never, ever, allow the release/display of such absolute garbage.
