If they did/do have a QMS and QA processes and people in place then they need to step it up a notch and issue an ncr to whomever is responsible for this and ask for a full RCCA. After all, and if I interpreted prior info correctly, isn't the DEWM CEO the hardest working CEO in the OTC? If so, this should be resolved very quickly
