I agree with most of what you said; however, a trial can "reach/meet its primary endpoint" early at interim analysis.

I think language gets in the way of some otherwise fairly straight forward concepts. I blame no one, but the scientific community should make these terms far more accessible to others.

Grant's mid-2016 IA date may be a typo. It makes better sense if his IA prediction was mid-2015.
