Gábor ..just a quick note .
Your posts deserve more attention then I have at present .
I would just note the following difference between 2011 and 2016 in my experience .

I have been switched from max dose Lipitor to max dose Crestor .
Crestor appears to be the better of the two in reducing lipid risk profile .
Kaiser is now very aggressive in follow up to ensure CAD patients get an annual flu shot.....i.e. Texts , emails etc etc at the start of flu season " please verify you have had a full shot as evidence supports benefit for high risk CAD patients " etc..

To me there seems to be a stronger drive to get CAD patients to lower BP , go onto stronger Statins , get flu shots etc , now then in 2010/11 .
