Remember JELIS was an Open Label trial (more vulnerable to bias ) with a heavy reliance on reported Angina.
Thats why I prefer to look at the hard numbers for MI and Intervention in JELIS ..not the total numbers

??? I am not aware of any data / table (aka breakdown) for MI and Intervention other than for the total.

It sound like you are mixing the "Cardiovascular history" (at baseline) with "Major coronary events" (during the study).

e.g.

Among the 1,050 patients with prior MI, the incidence of MCE in the EPA group (15.0%) was significantly lower than that in the control group (20.1%, adjusted HR 0.73, P=0.033, NNT =19)

The 15.0% and 20.1% had a heavy reliance on reported Angina, most of it were Angina.

I suggest you read about probe studies!

So you suggest the hospitals and ER departments skewed the data?
When the patients visited...
