Here's an indoor/outdoor record involving the company in question that I don't think can be matched.
From 2009 forward the company filed a total of 10 8k and 8k/a's. Every one of them involved an Item 4.01: Changes in Registrant's Certifying Accountant.

The PCAOB got involved at one point because one of their auditors employed the company's accountant to join in the audit of his own work.
I didn't make that up.
