The pianist and the artist had their disease for several years and had given up on their artistic skills. This usually is a sign of confusion and reasoning in their thought process. The patients eventually don't know where they live much less who is their friends and family. I would think the placebo effect would only be considered in the very early stages of the diseases. Just my opinion.
