Wy would the SEC grant an approval of confidentiality

The SEC wouldn't agree to permit certain exclusions in the 10K for confidentiality if the issue is to avoid any material adverse disclosure - that would obviously defeat the purpose of the 10K. This appears more likely due to disclosures related to the trials and the FDA that the company doesn't consider appropriate to make public as of yet, but is not consider adverse by staff at the SEC.

The extension for the filing deadline was not entirely unexpected since the company has filed late before, although not last year. I get the sense that the company is waiting for either the completion of the investigative report and/or some other relevant news, with at least the hope of reporting that information in or at the same time the 10K is released.

Management does seem to have some expectation that good news will break their way, but it's not entirely in their control for when that will happen.
