Hi flipper i have now come to agree with you on this:

2. It looks like the 149ers had it right, and the first interim for PFS had not occurred by October 15, 2015.

i am now speculating that there have been a fair number of indeterminate progressors, those that had 'progressed' initially but haven't progressed further during the 3 month assessment period to verify progression. i believe it is these indeterminates that are confounding the trial and they have been trying to negotiate a resolution to the problem for the last 6 months.
