Don't forget Aetna moved V from T2 to T3 end of 2014, before reversing themselves in Jan 2015 - at least for Medicare in my state, and we know coverage varies widely by state, even for the same insurer, for reasons I do not understand. I'm more worried about what changes Aetna will make in 2016, including raising rates - they didn't get Humana for free, after all, gotta pay for them somehow. For instance, as Comcast gobbled up companies over the years, cable rates skyrocketed.
