So the question is...

Where did Ameritrade get the shares they issued when NO other brokers had them? When both Paivis and DTC both clearly state there are no shares yet?

That's what I would ask 'em. And I'd be sure to get a last name too.

We might actually get 'em on this one. Their butts are hanging a mile out here.
