First, what is the date on that document? It indicates you have "one (1) year from the date hereof" to identify yourself and assert your right to receive a distribution.

Second, were you a holder of the old shares of WAMU back in February 2012 when they stopped trading?

Third, if the answer to the second question is "yes", did you submit the required documents (including the releases) back in January/February 2012? If not, it may be that the Court is giving those who failed to submit those documents one last chance to assert their right to a distribution.

Finally, does your account have any indication that you hold "escrow" interests? If not, and your answer to the second question is "yes", then you either did not submit the docs back in January/February 2012 or they were not processed.
