To the best of my knowledge, a form T trade usually happens when a MM gets a large order that he can't fill in one trade. The MM goes into the market and acquires the shares a bit at a time, then fills the order (or whatever lesser amount he was able to accumulate) in a single T trade, usually just after the market closes. If the order was for more than the MM could get today, you may see a repeat of this action tomorrow, etc. until the buyer gets what he wants.
