A copy of the Washington Mutual Inc. beneficial ballot claim form is required to be returned once the election has been submitted with an E*TRADE Service Associate. Any standing election will be cancelled if a properly completed ballot is not returned any time prior to the request of the Court.

A separate ballot is required per election for every account. Most ballots require pages 6, 7, Claimant Information, and W9. Your account number must be marked on each page of the document.
