I only received the K ballot yesterday.

But I called the Scottrade local branch (LA) and the service rep told me to drop by and they will have the PDF ballot for me to fill out. So I went there yesterday and filled them all out. They will fax the ballot back to the corporate office and I am going to call them Monday to make sure they got it.

You should call your local branch and see what if you can drop by and fill out those print ballot.
