Its a great read all 100 pages.
I can understand some reluctance from FDA in some of the studies (eg number of patients) but when you add them all up I believe they will decide on efficacy and the fact there has been not a single bad result should support OTC.
IMHO a lot of the delay may be around the warnings to be displayed on boxes and any inserts as you rightly say there are people who shouldn't use this.
