AMR101 and the new COMBO(lipitor+AMR101) pill would be priced both at branded prices(read high price). Keep in mind AMR101 would be patent protected.
So even if somebody were to make their own concoction of AMR101 and generic lipitor , it would cost as high as COMBO pill because AMR101 would be priced very high...it would be patent protected

Your scenario only impedes substitution if the price of AMR101 = the price of the COMBO. But at that price you're asking Pfizer to spend years developing a combination that they won't be able to charge for.

Otherwise, at any price where the cost of the combo > cost of AMR101, docs will substitute.

It's not that complicated.
