Regarding Form 4/A, line 5- I'm confused

Regarding line 5 "under explanation of responses" on form 4/A- I did a document search twice for John Salamon in the 8K.

If I missed the incorrect info which is entirely possible-these searches aren't full proof- where is it in the 8K? I can't find it.

I did though find the following:

Effective December 7, 2010, Michael Matvieshen replaced John Salamon as President, Chief Executive Officer and Director of the Corporation.
