The patient could have had no problem with her name being released. Its the company and surgeon at risk. But if they left the patient name visible , and blacked out everyone else's name who knows what people would think they know whats going on. So not blacking patient out the name all the way thru document i See as an error, and were more focused on concealing the identity of surgeon and company.
