Why didn't they black out her name at the end of the document? If they blacked it all out for legal reasons, they definitely would have made sure to cover all instances of her name. Who is Adriene and why isn't her/his name blacked out? According to the document, she referred Jacqui to this surgeon...so why is the referrer the only one getting a copy of this?
