The NYTimes has a long article on BOP’s and blind shear rams, focusing on what regulators knew about them and when they knew it:

A lot of the discussion in the NYT piece has been covered on this board, although I don’t recall anyone here talking about whether BOP’s ought to have two blind shear rams rather than one.
