From a few articles I've read, there was a remote valve control that BP decided not to install because US regulations do not require it. However, BP has installed this control on other wells because certain other countries do require it. Also, it was mentioned that Halliburton might also hold some responsibility due to their concrete pouring. Who knows if we'll ever know what really happened.
