I've had dealings with msha for several years, violations are issued for minor infractions such as not having a porta potty properly set up, no tag on a fire extinguisher, etc... as well as more serious offenses. MSHA assesses the operator based on criteria under CFR100, the opertor has 10 days to request a conference with the district msha office after a citation is issued and 30 days to contest the penalty after it is assessed. I have seen it take as long as a year to two years to resolve the fine. 25 citations should not be of a concern in itself only the severity of the offense. Many times the penalty is reduced as it makes it's way through the system.
