here's a link to my appeal letter. Keep in mind this was sent after I tried the generic for about 45 days. Their subsequent approval was because:
1- my TG's doubled
2- I reported SE's of diarrhea, and fish burps
3- Although they didn't admit it, I think they recognized that generics are not approved for CVD
4- I also, believe that in Texas when a doctor puts DAW they must comply???
