Mathematically, the above statement is not true if by "universally" you mean every single treatment-patient had a reduction.

All the above statement means is that the mean-change for the treatment arm is "sufficiently/very" different from the mean-change of the placebo. Here, "sufficiently/very" is relative to the distributions of the values of the two arms. So the mean-reduction is "very real" -- and not some random chance.
