Direct captures all those antigens from the inside out because it is injected into the area where neoantigens are hiding and is close enough to necrotic tissue to pick up antigens there as well. Once Direct begins the proper pro inflammatory signaling process the rest of the immune system begins to get involved. What is absolutely essential is that this proper signaling is maintained for the correct amount of time and that is done by proper spacing of treatments. Without this, the immune response begins to shut down too early and becomes more difficult to overcome during a restart. Sometimes there is enough of a humoral immune response that even improper spacing can be overcome to some extent because enough damage has been done to the tumor structure to provide ongoing access to targets and is especially likely in methylated patients where cancer repair mechanisms are hampered.
The more damage the more targets identified until all targets within a given tumor and in many cases surrounding tumors with similar composition are targeted and destroyed. When multiple tumors exist, injecting them too is important as there may be important hidden neoantigen targets within and protected by self antigens that need to be identified to eliminate all tumors and cancer cell clusters.
