PD, you make a very reasonable argument. My only thought on this is that the governing body responsible for reimbursement doesn't itself test out every drug or medical device to see the measurable benefit.

They rely on submitted clinical trial data to find out the measurable benefits. In the case of SWAV, it has been shown to clear CAC. In the case of Vascepa it has been shown to reduce cardiovascular events. Obviously the benefits are more immediate in the case of say SWAV (which you pointed out) but that does not apply to many if not most drugs. Heck if we simply look at statins and those that feel they offer great benefit, no one I believe would say that they offer immediate protection from CVD events.
